- We provide NDIS-related support and care services, including (but not limited to) Accommodation/Tenancy, Personal Activities, Transport, Community Nursing Care, Development of Life Skills, etc.
- The scope, cost, duration and any specific service plan will be agreed in writing (or electronically) between you (or your authorized person) and us.
3. Eligibility / Access
- You must be of legal age, or have a guardian/representative to enter into these Terms.
- Accessing certain services require you to satisfy eligibility criteria (for example being an NDIS participant, etc.).
4. Client Obligations
- You must provide accurate, complete and up-to-date information when requested (e.g. health information, contact details).
- You must follow the instructions of care staff, and cooperate in your care plan.
- You agree to pay any fees / charges as agreed in your service agreement.
5. Fees, Payments & Cancellation
- Fees for services will be as per your service agreement.

6. Intellectual Property
